Required Documents

1.The identification page of your passport OR other government-issued ID
2.Funding Documents (Refer to cost of attendance listed in the eForm)
3.If you are receiving funds from an individual sponsor (Example: Parent, Sibling, Friend, etc.), an Affidavit of Support listed in the eForm
4.Government- issued marriage and birth certificates for any dependents

Log in to eForms AND apply for your I-20 or DS-2019

  1. Go to https://ist.byu.eduhttps://ist.byu.edu and log in to our eForms portal
  2. Complete and submit the HomeBase course. (Currently, only I-20 requests must submit a HomeBase course.) You will then have access to:
  3. Select Initial I-20 from the Initial menu OR Initial DS-2019 from the J-1 Student menu
  4. Carefully read and follow the directions, entering the required information, and submit the completed eForm. You may save it and return if necessary.
  5. Submit dependent requests for any dependents who will accompany you.

Receive your I-20 or DS-2019 AND pay your I-901 (SEVIS) fee

  1. We will send your I-20 or DS-2019 to you electronically with our authorized electronic or digital signature and instructions for you to print, sign in ink, and keep with your documents.
  2. You must then pay your required I-901 SEVIS fee.

After being admitted, you must obtain either an I-20 to be an F-1 student OR a DS-2019 to be a J-1 student.
