Your Form DS-2019 is a Certificate of Eligibility
DO NOT let your DS-2019 program end date expire until you have completed your program at BYU.
Request an updated DS-2019 through our eForms Portal when you have changes to your circumstances, including but not limited to the following:
- Needing to extend your program end date
- Replacing a lost or damaged DS-2019
- Changing of sponsor/finances
- Adding or deleting dependent DS-2019s for spouse and/or children
- Changing your name
- Updating employment information
- Obtaining a travel signature
We strongly urge you to keep all of your original DS-2019s even after a new one has been issued to reflect updated information or transfer of schools. This is recommended for future government applications where proof of enrollment and employment are needed and/or for other work applications.
